Before one of my instructors
Before one of my instructors would teach me theory he wanted me to become a better sight reader because he feels it is easier to show you on paper and then explain what is going on so he directed me to purchase
"Music Reading for Bass" by Wendi Hrehovcsik
It is the same book they use to teach at M.I. and it is very easy to understand.
As far as a good book on how to use theory, maybe Wheat will chime in here but if you know your scales/modes and what notes over lap then you know where you can move from one to the next without to much dischord.

I cover some theory in Wheat's BassBook, especially in Book Two.
I'm not sure how much theory Patrick covers in his excellent Bass Guitar for Dummies. But, if you don't own that book already, I highly recommend it.
If you really want to get down and dirty with theory, you might want to pitch a wider net and take a look at music theory in general.
The problem with learning theory is that the term covers a lot of ground. It can mean any of the following things (or some combination of them, as they are often overlapping):
If you're into jazz at all, and can read standard notation, Mark Levine's The Jazz Theory Book would be a good start. I haven't read that one, but I really liked one of his previous ones that's now out of print.

Wheat´s on-line books are great.
But if you´re Iooking for paper version check out bass guitar for dummies. I found it usable, even though it wasn´t written in my mother tongue.

EDIT:
I would like to say, that get in to a band (if you aren´t yet). There is no theory in a world that would give you same thing that other musicians.

It's totally true: you
It's totally true: you learn more playing with other people than all the books can teach you. Definitely get books and read as much as you can, and practice as much as you can. But also find other musicians and jam with them. And try, if possible, to put yourself into a variety of different playing situations (some within and some just out of your reach). There are all manner of tricks that you learn from playing with others that would be hard, if not impossible, to explain in a book.
Here's one example: I can follow along on most songs that I don't know through a combination of knowing how chord progressions generally work and by knowing what the various chord shapes look like on a guitar. Even with a crappy mix, or on material I don't know well, I can play off the guitarist's (or the pianist's) left hand. I've never seen a book teach you that trick. But most bassists (and damn near every guitarist) knows how to do this.
Also, you ears get sharper. You get to where you can hear the changes and how your own notes fit against them. The theory you learn in books will help you understand what's going on. And diving in and doing it will help you understand the theory. It's symbiotic. (How's that for a ten-dollar word?)

you hit the nail on the head......
and just told how i play most everything...and my philosophy on playing....i play with anyone....and i love playing stuff i don't know or never heard more than i do playign stuff i have heard and played all my life. pushing yourself makes you learn so much more than playing in your comfort zone all the time.
